Please do not return any form to us by email as an attachment. At the insistence of our IT Department, we do not accept attachments, and your email will be deleted unopened. You must print your form, sign or initial it as indicated, and return it to us by mail or fax.

If you would like to allow someone else to use your unit/week, please fill out one of the guest authorization forms below. If you OWN the unit/week in question (you pay maintenance weeks to Canada House Beach Club for that specific unit/week), use the OWNER form. If you do not own the unit/week, please use the NON-OWNER form.
